I am looking to make an appointment. How do I start?

To begin the process of scheduling an appointment with us, you will need to complete an application before services can be rendered. Once your application is submitted, we will call you to schedule if you qualify.

Where can I find the application?

The application can be found by clicking "Click Here to Apply for Services," at the top of the webpage. Paper copies are available under the "Patients" tab or can be picked up directly at our office. If you would like us to mail you a copy, please call us at 724-841-0980 or submit the contact form below.

Do you take walk-in appointments?

Accepted Answer

Unfortunately, we are not a walk-in clinic. If you are experiencing emergent/life-threatening symptoms, please visit your local ER to be seen. If your symptoms are not emergent, please fill out the application and we can try and schedule you as soon as possible depending on availability. The application is on our website at the top of the page.

Question 4

What dental services do you offer?

Accepted Answer

We offer basic dental services. This includes extractions, fillings, cleanings, and x-rays. We do not offer crowns, root canals, dentures, or implants. If you are in need of services we do not offer, we can provide local clinics that offer these services on a sliding-fee scale.

Question 5

I am currently experiencing dental pain and I need to be seen ASAP. How can I be seen at the CHC?

Accepted Answer

We are so sorry to hear that you are experiencing pain and would be more than happy to assist you. We will still need a completed application before we can schedule you an appointment. Once we have your application, we will go over our dental schedule and try to fit you in at the earliest appointment. If your pain worsens, please visit your local ER to receive treatment.

Question 6

I need an appointment for my wisdom teeth. Is this a service you offer?

Accepted Answer

We would need to see you for an exam to determine if that is a service we can perform. We are not equipped for oral surgery but can service wisdom teeth on a case-by-case basis. Please fill out the patient application, and we will call you to schedule an exam.
