Who is Eligible?

To be eligible for services at the CHC, you must:

- Be Uninsured OR Underinsured

- Have countable income below 300% of the Federal Poverty Income Guidelines

- Not currently seeing physicians for the services requested

      - Ex. Having a PCP but requesting medical services

(We do not duplicate services and cannot see you while you are seeing another provider)

New Patient Process

To receive services at the Community Health Center, you will first need to complete an application.   

 

Once we receive your completed application, you will be contacted to schedule a new patient appointment.  If you do not hear from us within 48 business hours of submitting your application, please give us a call at 724-841-0980.

Patient Application

If you are in need of medical, basic dental, vision, or behavioral health care at the Community Health Center, you MUST complete an application before services can be provided. Once your application is submitted, we will contact you to schedule an appointment.
