If you need health care and are uninsured, you may be eligible for free care at the Community Health Clinic of Butler County.

We are not a “Walk-In” urgent care facility, all new applicants must complete the Clinic’s intake process to qualify for services before you can been scheduled for a medical appointment.

On average, there is a 4-6 week waiting period for new applicants to be seen by a medical practitioner at the Community Health Clinic. Apply today so we may assist you!


Who is eligible?

  • Butler County Residents
  • Individuals who have no private or government-funded medical insurance
  • Aged 19-64
  • Yearly household incomes no greater than the amounts listed below:

To request services please visit the Community Health Clinic to request a “New Applicant Forms” packet. 

These forms will not be mailed out. 

Upon arrival to the Community Health Clinic you will be asked to complete a One-Page New Applicant Form.

You may complete this form in advance by printing it from our website.

What To Expect

1.    After your One-Page New Applicant Form has been completed, you will be given the New Applicant Packet as well as a checklist of documents that must be provided before services can begin. You must visit the Clinic to obtain the New Applicant Packet, this form is not available on our website and cannot be mailed.

2.    We ask that you take the New Applicant Packet home with you to complete at your convenience. Once completed, return the New Applicant Packet and additional financial documentation to the clinic. You may also mail, e-mail or fax the documents to the clinic for review. Click here for HOURS/CONTACT INFORMATION

3.    Once you have completed the intake process and submitted all required documents, you will be notified to discuss your eligibility for services at the Clinic. A New Patient Physical will be scheduled with one of our medical practitioners provided you qualify for services.

To expedite this intake process please call to schedule an appointment with an Intake Volunteer. Intake Volunteers are available to assist with any questions you may have completing the forms and/or copying the required documents.

To expedite the intake process you may visit the clinic during the days/times we normally have Intake Volunteers available:

INTAKE VOLUNTEER HOURS

Monday (10am – 2pm)

Wednesday (12:30pm-2:30pm)

Thursday (4:30pm – 6pm)

Friday (10am – 2pm)


You may visit the Community Health Clinic during these times and meet with an Intake Volunteer if a volunteer is available. Should a volunteer NOT be available, you will be asked to follow the steps above OR schedule an appointment for assistance.
