All CHCBC patients are required to complete an "Annual Review" every year to confirm their eligibility for continued services. To complete this process please select one of the options below:
1. Print the Annual Review packet --> Click Here
2. Visit the CHCBC to request an “Annual Review Packet”.
3. Call to schedule an appointment with a volunteer to assist you in completing the Annual Review process. Meeting with a volunteer best ensures the completion of this Annual Review process!
