If you need health care and are uninsured, you may be eligible for free care at the Community Health Clinic. To receive free preliminary medical services you must first be determined eligible for care.

Butler County Residents
Individuals who have no private or government-funded insurance
Aged 19-64
Yearly household income no greater than: - Individual...................$21,660
- Two People................$29,140 - Three People..............$36,620
- Four People................$44,100
*Add $7,480 for each additional member
Patients are seen by appointment only. Your first appointment will be to establish that you are eligible for service. When you are accepted you may make an appointment for a medical visit. Please complete the online application and an eligibility screener will contact you to complete the process for services. Phone screening times are: 1:00-4:00 Tuesdays, 9:00-12:00 Wednesdays, and 4:00-7:00 Thursdays.
Please note:
The Community Health Clinic is not a walk-in clinic.
Volunteer health care providers and eligibility screeners are only available by scheduled appointment.
We will not duplicate services already available in the community. Our case manager and trained volunteers will help guide those eligible for other services to the appropriate agency.
There are no narcotics or cash at the Community Health Clinic.

Upon receipt of all of your information, your file will be reviewed for eligibility, normally within two weeks. When scheduled for an eligibility appointment please bring the following information:
Photo ID
Proof of address (driver's license, utility bill, rent receipt, etc.)
Proof of income for household. Household is defined by the number of dependents claimed on your most recent tax statement. Income: All sources such as: employment, self-employment, unemployment compensation, Social Security Income, Supplemental Security Income, Social Security Disability Income, pension, veteran's benefits, welfare, child support, food stamps, etc. (Please call for more information).
Current W-2 Form for most recent tax year.
Current 1040 Tax Form (Copies can be obtained by calling 1-800-829-1040)
